Financial Responsibility Policy:

Scioto Urgent Care is committed to providing you with the best possible care. If you have insurance, we are anxious to help you receive your maximum allowable benefits. In order to achieve these goals, we need your assistance and your understanding of our policies and practices.

If you have insurance, a current insurance card and any copay is due at the time of service. We will be happy to process any insurance claims for you and we accept insurance assignment with our in-network providers. At this time our in-network providers are as follows: Aetna, Anthem Blue Cross and Blue Shield, Bureau of Workers Compensation (BWC), Medical Mutual of Ohio, Medicare, OSU Prime Care, and United Health Care. For out-of-network insurance, we require a $50 deposit. Your ultimate responsibility is based on your insurance provider’s out-of-network urgent care benefits. We do not accept any form of Medicaid.

Ultimately, your insurance is a contract between you and your carrier. We are not a party to that contract. Any service that is not covered by your insurance company, for whatever reason, is your responsibility.

Personally-Identifiable Information:
Scioto Urgent Care typically receives specific data about its Web site visitors only when such information is provided voluntarily, such as when our visitors request information, purchase or enroll for services, provide resume information for employment opportunities, or send us e-mail. Of course, some of these activities require that you give us information, such as when you make a purchase, use a credit card to pay for services, submit your resume, or request certain types of information. When you provide personally-identifiable information to Scioto Urgent Care through our Web site, it will be used to fulfill your specific request. In most cases, you will be given the opportunity to select whether you do, or do not, want Scioto Urgent Care to use this information for additional purposes. You may also request that Scioto Urgent Care not use your information by sending an e-mail to info@sciotourgentcare.com, however, Scioto Urgent Care reserves the right, in its discretion, to send you bulletins and other important information about your Scioto Urgent Care services. Absent any instructions from you, Scioto Urgent Care may use information you provide to inform you about additional services and products offered by the Scioto Urgent Care family of companies, Scioto Urgent Care authorized agents, and other goods and services providers with whom Scioto Urgent Care has relationships and whose offerings might be of interest to you. Scioto Urgent Care will not, however, sell or trade your personally-identifiable information unless we are authorized or legally required to do so, or in the case of imminent physical harm to the visitor or others. On those Scioto Urgent Care sites where you may provide Scioto Urgent Care with credit card or other ordering information via the web, Scioto Urgent Care protects and secures this information by employing commercially customary web- based security and encryption protocols, examples of which include Secure Socket Layer (SSL) and Secure Electronic Transaction (SET). On those sites where you voluntarily offer any feedback, data, answers, questions, comments, suggestions, ideas or the like, Scioto Urgent Care will treat that portion of the information as non-confidential and non-proprietary and, except as otherwise expressed in this privacy statement, Scioto Urgent Care assumes no obligation to protect such information from disclosure.

B. Non Personally-Identifiable (Generic) Information:
In general, Scioto Urgent Care gathers some generic information automatically. Generic information does NOT reveal the identity of the visitor. It usually includes information about the Internet address assigned to your computer, the number and frequency of visitors, and the Scioto Urgent Care sites visited. Scioto Urgent Care gathers this information for the limited purpose of determining customer service and Web site needs. We accomplish this by using certain technologies, including “cookies” (a technology that can be used to provide the visitor with tailored information about Scioto Urgent Care services). Scioto Urgent Care does not combine information collected in this way with any personally-identifiable information. Additionally, Scioto Urgent Care Web sites contain links to sites that belong to third parties unrelated to Scioto Urgent Care. Scioto Urgent Care cannot protect any information you may disclose in these sites and recommends that you review the privacy policy statements of those sites you visit.

D. Exceptions and Limitations:
Notwithstanding the foregoing and in compliance with applicable laws, Scioto Urgent Care (i) cooperates fully with state, local, and federal officials in any investigation relating to any content (including personal or private electronic communications transmitted to Scioto Urgent Care) or purported unlawful activities of any user of the Service, and (ii) takes reasonable measures to protect its proprietary rights. For the limited purposes of accomplishing such cooperation and measures and in compliance with applicable laws, Scioto Urgent Care may be required to disclose personally identifiable information. In addition, Scioto Urgent Care may elect to monitor the areas of communication of any kind (i) to satisfy any law, regulation, or government request; (ii) if such disclosure is necessary or appropriate to operate Scioto Urgent Care; or (iii) to protect the rights or property of Scioto Urgent Care or others. In connection with the potential sale or transfer of any of its interest in www.sciotourgentcare.com, and other sites owned by the company, Scioto Urgent Care reserves the right to sell or transfer your information (including , but not limited to name, address information, and other information you provided to Scioto Urgent Care) to a third party that (i) concentrates its business in communication products or services; (ii) agrees to be Scioto Urgent Care’s successor in interest with regard to the maintenance and protection of information collected and maintained by Scioto Urgent Care; and (iii) agrees to the obligations of this policy statement.

General Policies:

To enable us to continue with the highest quality of care we need your assistance and cooperation with the following policies.

  1. All outstanding balances have to be paid prior to your next appointment. If you have any billing issues please call (614) 789-9464.
  2. A 72-hour prior notification is required for all referrals and prior authorizations. This office will not fax or mail these referrals to you or any Health Care Provider/Institution. It is your responsibility to know the details OF YOUR health plan (co-pays, referrals, etc.)
  3. This office will not mail, call in or fax any prescriptions to you or your mail order pharmacy. Requests for prescription refills will not be taken on weekends or holidays
Smoking Policy:

The Scioto Urgent Care is a smoke-free facility. We appreciate your compliance with our smoke- free health standard.
